Registered Nurse (RN) Job Description

Qualifications:

 Must maintain license by the Texas State Board of Nurses.

This job description is designed to clarify your responsibilities as the person who has agreed to work as a Registered Nurse (RN) for Live Life Freely, Inc.

The Registered Nurse (RN) must be available in person or via telecommunication, participate in activities relevant to professional services furnished; including the development of qualifications and assignment of agency personnel and assure that the client’s Individual Plan of Care (IPC) is executed as written. Assure that a reassessment of the client’s needs is performed by the appropriate health care professional; when there is a significant health status change in the client’s medical condition, at the physician’s request or after hospital discharge.

The duties and responsibilities also include, but are not limited to the following:

  1. Transcribe physician’s orders.
  2. Complete and update Consumer’s Annual Physician’s Order.
  3. Complete Quarterly Reports on each Consumer.
  4. Update Medication Log with new, changed or discontinued medication.
  5. Make all doctor appointments.
  6. Audit MAR Log and Medication Control Log for any potential medication errors. All errors must be reported to the Program Manager for corrective action.
  7. Collaborate with physician, physician’s staff and Interdisciplinary Team (IDT) regarding client’s medical condition, type of treatment and medications.
  8. Call in all refills to the pharmacy, if no refills are on the bottle please contact the physician’s office via fax for refill request.
  9. Attend staffing meetings.
  10. Complete Provider Logs as needed for billing purposes.
  11. Except responsibility for being on call for any medical emergency or to answer any medical questions that the staff may have regarding the Consumer’s medical needs.
  12. Maintain Nurse’s notes to record communication with all parties involved with the well being of the Consumers.
  13. Maintain a professional attitude at all times when communicating with the Consumer’s, physicians and or their staff, the Consumer’s family and all of Live Life Freely staff. (No exception’s)
  14. Occasional visits at work-sites/homes as deemed necessary.
  15. Conduct medical training for staff.

In Addition:

The Home and Community-Based Services Program Provider shall have evidence that nursing services are delivered in compliance with the required definition and consist of the performance of health care procedures and monitoring consumers’ health conditions including:

  1. Administration of Medication
  2. Monitoring the individual’s use of medications
  3. Monitoring health data and information
  4. Assisting the individual to secure emergency medical services
  5. Make referrals for appropriate medical services
  6. Performing health care procedures ordered/prescribed by a physician/medical practice or law to be performed by licensed nursing personnel.
  7. Delegate and monitoring of tasks assigned to other service providers as stated in accordance with state law.
